The simple answer is, hardware based fax modems (including the USR ones) are generally well supported under OS/2, including solutions up to 64 ports per system.

Most software based modems will not work. One exception is the Mwave. In the case of USR, the 5661 and 5699 series/versions will not work - if it's labeled in some fashion or another "Win Modem" or similar, chances are pretty bad.

Initialization strings for the ones that work (as well as indication of some of the ones that do not) are listed in the link DougB provided.

Why Eugene provided a link to a site page that has no USR modems listed, baffles me - so you can skip his advertising link.
